How To Get A Flat, Toned Stomach

One of the most common fitness questions this time of year is “How can I tone my stomach for swimsuit season”? Typically, people believe that the guaranteed quick route to obtaining a rock-solid six-pack is a path packed with hundreds of sit-ups or abdominal crunches. Wrong!
The reality is that even if you do crunches every day you aren’t guaranteed to get the stomach you desire. Getting those highly sought after, toned abs requires more work than just abdominal exercises. Plus, as far as stomach exercises go, sit-ups or crunches alone are not the solution.
We won’t go into detail about the muscles that make up the abdominal wall, but it’s good to know the basic information. At a high-level the abdominal/trunk area consists of 5 major muscles. It’s necessary that all of these muscles be exercised. It’s also important to utilize different types of training techniques like concentric, eccentric and isometric.
In addition to training those muscles, it is imperative that you also reduce the fat in your stomach area. If you don’t decrease the fat in this area, then you’ll never see well-defined abdominal muscles not matter how long and hard you train them.
The key to reducing body fat is a comprehensive workout program that consists of cardiovascular, strength, and flexibility exercises. Plus, don’t forget a healthy diet. Good examples of cardio exercises are: walking, swimming, aerobics, and jogging. Strength training can be done with dumbbells, resistance bands and even just your own body weight. Flexibility can be as simple as a few stretches held in place for about 20 seconds.
Now, back to the specifics of abdominal training. Traditional crunches can be a part of your abdominal training, but should be limited and certainly shouldn’t be the only part. To most effectively work your stomach area, it’s necessary to include about 80% rotational work. Rotational exercises are those that include twisting-type methods. Simple crunches do not fall into the rotational category because you simply lift your body straight up and down.
To most effectively train your abs, incorporate a lot of variety, including different positions and equipment. This will allow your muscles to continually be challenged, which is what will help make them stronger and more defined. Remember that you don’t have to lie on your back in the supine position to work your stomach area. There are hundreds of different ways to work them, including exercises in a standing position, on your side, raised on a ball, hanging from a bar, etc.

Don’t ever pull on your neck or head
Don’t allow your legs to move, they should remain still — let your abs do all the work
Try to keep your belly button pulled in toward the floor throughout the entire move.

To get you started, below are a few sample exercises* with instructions. Start working out today, and you can show-off your toned abs in time for summer.
Basic Crunch: Lie flat on your back with your knees bent and feet flat on the floor. Place your hands lightly behind your head for support. Using your abdominal muscles lift your shoulders a few inches off the ground, pause briefly and return to start position. Complete at least one set of 10-12 reps.
Standing Towel Circle: Stand tall and hold a small towel (or resistance band) taut overhead. Contract abs and slowly draw a large, wide circle over your head and around your torso with your hands. Keep towel pulled taut throughout. Return to beginning position and reverse the circular direction. Complete at least one set of 8-10 reps in each direction.
Oblique Twist With Ball: Begin by sitting on the ball. Walk feet forward until ball is resting under your back — body is now parallel to the floor. Place hands lightly behind head for support. Using your abdominal muscles slowly crunch up lifting your shoulder blades off of the ball and rotate left shoulder toward right hip. Return to starting parallel position and repeat then switch sides.

How Do I Gain Weight?

Try asking people how to gain weight and you’ll likely see some bewildered faces staring back at you. For the majority of people, hearing that question is akin to hearing the questions, “How do I stub my toe?” or “How do I run out of gas?”
Those confused faces not withstanding, the question is a legitimate one and one that frustrates those who find themselves on the light end of the scale. While those on the heavy end can’t get through a commercial break or flip through more than five pages of a magazine without someone offering up a solution to their weight problems, it can be a lonely struggle for those who want to put on weight.
If you are lucky enough to get a reasoned response to your question, it will probably involve the words “eat more.” That, despite being obvious, is great advice. Quite simply, to gain weight you will need to up your calorie consumption to the point where you consume more calories than you expend. Couple an increased caloric intake with a little weight training and you have the recipe for healthy weight gain.
You may think you eat a lot, even enough to keep pace with your more weight-furnished friends, but you are probably over-estimating your intake. After a trip to the doctor’s office to rule out medical causes for your inability to bulk up(thyroid disease and other medical problems can hamper weight gain), a first step to designing an effective weight gain program is to journal your eating habits. Counting calories for a week will give you an accurate view of your diet.
From there, increase your daily caloric intake by about 300-500 calories until you start putting on the pounds. Keep in mind though, while your caloric intake will directly influence your bodyweight, it will be other factors like the types of food you eat and your weight training regimen that decide the type of weight you are putting on. So if you are after an aesthetically pleasing weight gain, that of lean muscle weight as opposed to just some extra body fat to lug around, it will be important to pay attention to these factors.
To get the most muscle out of your weight gain, avoid the junk food and focus on eating whole foods. A good weight gain diet should be composed of 30-50% protein, 20-50% carbohydrates and 20-40% fat (the majority of which should be essential fatty acids). Different ratios within these ranges will work differently for different people. Keep up your food journal and experiment to find the ratios that work best for you.
Providing your body with the materials it needs to build muscle is only one piece of the puzzle. A weight training program designed to give the body a reason to add some muscle will be very important to achieving your goals. Your workout routines should concentrate on compound weight lifting exercises (those that involve multiple muscle groups like the squat or bench press) with weights that allow you to do 6 to 12 reps per set. Higher rep ranges will tone your muscles but may not sufficiently signal the body into muscle growth. Try to either increase the amount of weight lifted or the reps completed with each workout.
Don’t go overboard with your workouts. Two or three one-hour workouts per week done with intensity should do the trick. Too often those that have trouble gaining weight (hardgainers) spend too much time in the gym. This can be counterproductive in two ways. First, it means you are burning off a lot of the calories that could otherwise be used to build muscle and, second, you can over-train your muscles by working them too hard, basically making them unresponsive.

Heel Pain

Painful heels are the number 4 conern bringing patients into the offices of many family doctors and the number 1 concern bringing patients to the offices of podiatric physicians (foot doctors).
Heel pain can have many causes but the vast majority is caused by plantar fasciitis. Plantar means, “bottom of the foot.” Fascia is a ligament or “bundle” of ligaments. The plantar fascia is the thick ligament that helps to hold up the foot and provide spring in our step.
Plantar fasciitis, pronoucned (PLAN-tar FASHEE-itis) is an inflammation of the plantar fascia and causes more than 90% of heel pain among adults in the US.
Plantar fasciitis can be acute, that is, as simple strain of the ligament but often is chronic, hanging on for months if not years. Why does that happen? The answer is poor foot mechanics, the foot sinking down too far alllowing the plantar fascia to overstretch with each step taken.
If the plantar fasciitis is acute, that is, a sprain of the plantar fascia then it is basically treated as a sprain, with anti-inflammatory drugs, ice, rest, possibly physical therapy. If chronic, the poor foot mechanics need be addressed.
Foot mechanics are changed by use of specially molded shoe inserts known as orthotics. Someone with plantar fasciitis needs an orhtotic designed to releive strain on the plantar fascia. Orthotics are often confused with arch supports. Arch supports, by holding up the arch can remove some of the tension from the plantar fascia. Orthotics, on the other hand, do most of their work on the heel and ball of the foot repositioning the foot for maximized function.
What can you do before you see the foot doctor? First, try doing your own version of deep tissue massage by rolling a frozen cola bottle or can from the heel forward into the arch. Do it gently. Do stretching but the key to good stretching is not to stretch too hard so generally avoid weight bearing (standing) stretches but sit on a soft surface like your bed and pull the foot backward on the leg as far as it will go, holding for 20 seconds and relaxing for 5 seconds.
Each 25 second “set” can be repeated 5 times and you have invested about 2 minutes in giving yourself a lot of help.

A note from your doc may be all that is required in most cases and most doctors are happy to oblige.
Orthotics, by treating the cause of the problem, lead to the cure bette than 90% of the time. A small number of patients have waited so long that the plantar fascia has become thickened and filled with scar tissue and are not helped by “conventional” means. Those are the patients that have, traditionally, required surgical treatment in which theh plantar fascia is cut off the heel bone. Luckily, most surgery has been replaced by a relatively new mode of treatment, ESWT or Extracorporeal Shockwave Therapy. ESWT involves the application of multiple shockwaves to the diseased tendon or ligament and has an approximately 85 to 90% success rate. Keep in mind we are talking about this success rate in patients who are “tough cases,” that is, already had the conventional treatment.
The ESWT machines look like miniature renal lithtripsors (kidney stone crushers). There are virtually no side effects to ESWT other than the price as only about 30% of insurance companies are paying for it. They realize that it is less costly and safer than surgery but also know that many more people who would avoud surgery would have no problem gettting ESWT so the voume of services would go up.

Ahhh, the holidays… a time of friends, family and for all too many people…

Chocolate

All of the above beverages can increase the acid production in your stomach and give you heartburn. Instead try substituting hot cider (in moderation) for the coffee or tea, and non-alcoholic beer or wine for your other favorite alcoholic beverages.
Hors d’ Oeuvres
Spicy and fatty foods are notorious for generating stomach acid production, and no place are these foods more conveniently and neatly presented than on the hors d’ Oeuvres trays and tables at holiday parties.
Foods to look out for include cheese, salami, beef sticks, and, of course, spicy buffalo wings. Instead, stick with pretzels, apples, or carrots and broccoli dipped in low fat ranch dressing.
Brunch Food
Lastly, another meal that friends and family share together during the holidays–and one that’s loaded with heartburn traps and snares–is brunch.
From the coffee, to the eggs prepared in butter and garnished with bacon or sausage, to the donuts and fruit juices, brunch offers heartburn foods at every turn. Even here, just a few modifications can result in a very satisfying meal followed by a pleasant, heartburn free day.
Here are a few tips for making your brunch selections…
Egg white omelets are better than those made with whole eggs, and if you like to have some sort of bread with breakfast, try a whole grain bagel with low fat cream cheese.

Handling Problematic Physicians

Medical group managers must frequently deal with problematic physicians and the resulting negative organizational dynamics. It costs many practices valuable time, energy and money. One of the most important skills in handling difficult physicians is how to manage conflict.
Physicians come in all shapes and sizes. Some know business, many don’t; some know how to lead people, most don’t. But that’s why they have you, to help them with those things. Primarily, physicians want to practice medicine, period. When forced to practice medicine in the real world, they largely view the other things as distractions. Just as myopic as bottom-liners who forget they must work with people to improve the bottom line, many docs forget they must work in a peopled system, too.
In my experience working in and consulting to hospitals and medical group practices, problematic physicians behave in ways that fall into roughly three categories:
Oppositional-defiant (being argumentative, acting out, refusing direction, etc.)
Silent-superior (a form of passive-aggressiveness)
Weak-helpless (another form of being passive-aggressive)
Here is a practical model of how to deal with a physician in such situations:

getting defensive,
counterattacking,
leaving the field (“I’m not going to address that”), or
changing the subject (“Well, look at how much vacation you take!”).
Any of the above is basically used to “win” an argument, not resolve it. As an effective manager of conflict, you must get your ego out of it: give up winning in favor of resolution.
Nipping conflict in the bud-
Prolonged arguments predominantly involve statements, or sarcastic questions masking statements. However, statements alone don’t work in resolving disagreements because they don’t address the two factors that start arguments: inaccurate and/or incomplete information. So the key is to address these two factors.
Additionally, you need to hold the line on getting defensive, counterattacking, leaving the field or changing the subject.
Of several possible ways to curtail and resolve conflict, one most effective is a process I call “Data Gathering.” Not a touchy-feely approach, Data Gathering is a practical method to both defuse anger and begin to resolve an issue. Here’s the abbreviated version:
Step 1) Only ask questions…gather data…listen. Just try to see how the physician views the situation. Don’t respond with your view (prevents getting defensive, counterattacking, etc.). Only clarify, if necessary.
Step 2) After you feel you understand how the physician “ticks” about the issue, ask “Is there anything else you think I need to know about this?” At her conclusion, say you’ll give the matter more thought. This allows the physician to feel she’s been heard, with her points taken seriously enough by you to devote your added consideration. We all want our day in court. This serves that important purpose and cools things down.
Step 3) At the next meeting, thank the physician for her ideas and propose a solution. Again, use mostly questions, not statements, to respond to any objections. Using this process, a mutually agreed resolution will most likely bubble up in time, with a fight or power struggle avoided.

Good Food/Bad Food What’s Left to Eat?

What’s Good Today is Bad Tomorrow: What Can I Eat?
We’ve entered the Twilight Zone when it comes to the multitude of diets being promoted today. Starting with the Atkins Diet, then the South Beach Diet, now the Hamptons Diet and more. All higher in protein, lower in carbs, but the distinction should be quality of carbs, not singling out one nutrient entirely. If you are on the Atkins Diet, South Beach Diet or any other variation of a high protein/low carb diet simply adjust from eating low quality carbs like refined flour and sugar products (think if it comes in a box, it’s likely low quality) to eating more whole food products like fresh vegetables and fruits – yes fruits.
Apples vs. Apple Jacks – You be the Judge
I know the traditional Atkins Diet doesn’t advocate much fruit (too high sugar) but think about that for one minute. My strong belief is that an apple is a good food, a bowl of Apple Jacks cereal may not be on an equal level. One is highly processed sweetened by added white sugar and corn syrup, and one is natural, plucked from a tree and sweetened by the sun. Which would you choose? Don’t shun fresh fruit for the sake of following your low carb diet to the letter.
Eliminating healthy, wholesome foods is not the best way to learn to eat better, but severely cutting back on the frequency of eating highly processed foods is. I saw a site which called it GM or MM: God Made or Man Made. If you think of those terms when you go to choose your foods, it starts to make more sense. No one says you shouldn’t eat chips, or whatever strikes your fancy, but make them a treat – and eat the GM foods more often.
Common Sense Diet
Common sense will answer the question about what to eat. If you are on Atkins, South Beach or any variation of low carb diet, avoid processed foods, not natural foods. Stop using “instant” breakfast, and cook whole rolled oats for instance. Sure you might have to get up 10 minutes earlier, oh well. You’re worth it!

The person who refuses to eat fruit because they believe it is too high in sugar, probably does eat cookies, crackers and sugary cereals. They might even drink artificially flavored and sweetened drinks, but they refuse to eat a natural food, grown from our earth? That makes no sense, if you think about it. It will come to pass – nothing but your local farm will be allowed to sell meat because the big farm industry cannot guarantee safety of the food supply. Meat will become much more expensive because when they can no longer mass produce it, there is nowhere for prices to go but up. So do we whine and cry and moan about our misfortune or do we start to think of meat as something to savor and enjoy like the Sunday roasts we had years ago? We never ate meat every day then – and we weren’t so fat either. We simply didn’t eat as much processed foods. Most of us had moms at home cooking us dinner, making our lunches and even fixing our breakfasts.

Glutathione for a Healthier Pregnancy

All parents-to-be nurture the dream of a healthy pregnancy and baby.
But the modern environment and diet is deficient in many factors essential for the health of mother and fetus. One of those factors is antioxidants.

Most substances or factors which cause birth defects (teratogens) are known to exert their embryotoxic effects because they cause oxidative stress.
The human placenta possesses a significant amount of glutathione S-transferase (GST) capable of detoxification or activation of drugs and pharmaceuticals during the critical period of organ development in the fetus.
Some drugs are known to cause birth defects in the growing fetus by generating free radicals, and depleting GSH stores.
In the early embryonic stages, the fetus is sensitive to the toxic and teratogenic effects of chemicals, whereas it is sensitive to carcinogenic effects during late fetal stages.

Glutathione and other antioxidants attenuate oxidative stress in pregnant women with inflammation or maternal conditions like diabetes and pre-eclampsia, and in fetuses at risk for developing cystic fibrosis.
Supplementation with glutathione precursors and antioxidants can decrease the incidence of birth defects and protect both mothers and the fetus from the damaging and possibly fatal consequences of pregnancy complications.
Glutathione (GSH ) also prevents or minimizes the oxidative stress that occurs during labor and the birth process.
Perinatal or birth asphyxia/hypoxia (deprivation of oxygen supply to the brain) in preterm deliveries and labor can lead to cerebral palsy, respiratory distress syndrome, irreversible brain injury, and permanent neurological and intellectual handicaps.
Administration of the glutathione precursor, N-Acetyl-Cysteine (NAC), to the pregnant mother partially prevents oxidative stress during the birth process in premature infants.
Currently, the American College of Obstetrics and Gynecology advises all pregnant women to take a prenatal vitamin containing antioxidants.
In addition, they advise eating lots of fresh fruits and vegetables, the best sources of antioxidant protection.

Glucosamine – The Arthritis Cure?

Glucosamine sulphate has been around a long time now and as each year goes by more and more claims are made telling us about this miracle joint lubricant. Are all the claims true? Will it cure Arthritis? What is Glucosamine anyway? And what about Chondroitin?
Glucosamine is a naturally produced amino sugar which is found in small amounts in foods. It plays an important role in maintaining the cartilage gel-like material between our joints. The body also produces a carbohydrate called Chondroitin, which is thought to promote water retention and elasticity as well as blocking the enzymes that break down cartilage.
As we get older the body’s ability to manufacture and synthesize Glucosamine and Chondroitin decreases. This probably contributes to the joint problems we have all come to associate with growing old, a fact that health food companies did not take long to latch on to.
Although studies have been carried out in numerous countries to try to prove conclusively that Glucosamine is effective in treating arthritis and joint problems there have been an equal number of questions raised about the methodology of many of these studies. One such study, in Europe, took X-rays to measure the size of the gap in the knee joint before and after taking Glucosamine. Even though the results showed that the size of the gap was significantly larger, in a group of people taking Glucosamine compared to a group taking NSAIDs, critics said that the study was not large enough to draw firm conclusions. They also claimed the X-ray evidence was too difficult to interpret.
Many Vets and pet owners have been using Glucosamine to treat joint problems in horses and dogs for a number of years now. They swear by the effectiveness of this form of treatment for their animals. While there is no placebo effect in animals it is equally difficult to find well documented, and conclusive, scientific evidence to confirm the effectiveness of Glucosamine.
While there might be limited good, accredited, scientific proof as to the effectiveness of Glucosamine there is an abundance of people as well as pet owners who swear by the effectiveness of Glucosamine. Even though Glucosamine is often used in combination with other supplements, or treatments, the general feeling among most users is that it does help. Results of empirical studies in various countries have shown that arthritis sufferers report significant improvements when taking Glucosamine supplements. The same is also true for people who have other types of joint injuries or back problems. Some countries now sanction Glucosamine as a treatment for people with mild to moderately severe osteoarthritis.
To date no study has found any serious side effects from either Glucosamine or Chondroitin when taken as a supplement in humans. However people with diabetes are advised to keep a check on their blood-sugar level. While there have been no reports of allergic reactions to Glucosamine, since it’s made from shellfish shells, it may not be suitable for people with seafood allergies. It may also increase a person’s daily salt intake level – something people with high blood pressure may want to watch. Chondroitin on the other hand may sometimes cause bleeding in people with bleeding disorders or taking blood-thinning drugs. Like many supplements insufficient data is available about the long term effects and hence it should be not be taken by children, or pregnant or breastfeeding mothers.
The United States Food and Drug Administration embarked on a study involving 1500 people earlier in 2005 to determine the effects of Glucosamine and Chondroitin on people with arthritic problems.
